How this document is used well
DAP (Data, Assessment, Plan) collapses SOAP’s first two sections. BIRP (Behaviour, Intervention, Response, Plan) is favoured where funders want the intervention and the patient’s response to it made explicit. GIRP substitutes Goal for Behaviour, tying every note to the treatment plan. Choose the one your setting requires and use it consistently.
