Psychodynamic: the pattern arrives in the room

A focal theme, a defense operating in real time, and a transference moment you can either use or step over. The session where the work stops being about his week.

Psychodynamic TherapyintermediateSession 11 of 20 · Brief dynamic therapy~50 min

What you are practising

  • 1Hold a focus rather than following the material
  • 2Confront a defense without confronting the person
  • 3Work the triangle of conflict from defense toward feeling
  • 4Recognise a transference moment and judge whether to use it
  • 5Notice the countertransference before acting on it

Briefing

The frame

Twenty sessions, agreed at the outset, ending in nine. The focal theme, stated aloud in session three and accepted by him: "You learned that you are only tolerable when you are producing, so you produce — and then you are exhausted and quietly furious, and nobody knows."

Where you are

Sessions 4–10 established the theme in his work life and in his marriage. He has not brought his father since session six, and the sessions have become slightly comfortable. His PHQ-9 has moved 17 → 11 and stalled.

The case

Before you begin

What Praxical.Psych is, and is not

Praxical.Psych is an educational tool. It teaches the theory, evidence base and clinical method of psychotherapy at the level of an advanced survey course, and it lets you rehearse a complete session in a simulator.

It is not medical, clinical, psychological or legal advice. It cannot diagnose or treat anyone, and it does not state the law where you practise — requirements differ by jurisdiction and change. Nothing here creates a clinician–patient or solicitor–client relationship.

  • Every patient, transcript and case in this app is fictional — a composite written for teaching. None depicts a real person.
  • Praxical.Psych is not therapy, not a substitute for supervision, and not a credential. Competence to practise comes from training, supervised hours and licensure.
  • Scores from the instruments here are for learning how they behave. They are not a diagnosis and not a risk assessment, and must not inform a decision about a real person.
  • Do not enter real patient information. Everything you type stays in this browser, but that is not the same as a compliant clinical record system.
